13 symptom items (+ 2 follow-up items: co-occurrence, impairment). Yes/no symptom items (summed 0-13) plus a co-occurrence item and a 4-point impairment item; positive screen = >=7 symptoms AND co-occurrence AND moderate+ impairment (self-report questionnaire)
