Hypochondriacal attitudes, fears and beliefs and abnormal illness behaviour; 9 subscales (e.g. worry about illness, concerns about pain, health habits, hypochondriacal beliefs, thanatophobia, disease phobia, bodily preoccupation, treatment experience, effects of symptoms)

Length and format

29 (27-29 scored across the 9 subscales depending on version). 5-point frequency (no-most of the time); subscale + total sums; higher = more illness-related concern (self-report questionnaire)
